PADI Diver Medical Questionnaire
The PADI Open Water Diver course is the first scuba certification level and is the most popular scuba certification which allows you to dive anywhere in the world, up to depths of 18m (12m Junior). To take part in this course students must be at least 10 years old and be confident in the water. No previous dive knowledge is required.
The course consists of an element of course theory (e-learning), 5 confined water dives (1 being dry suit orientation), taken over 2-3 pool sessions, followed by 4 open water dives, typically undertaken over 2 days at an external dive site (fees apply)
COURSES START MONTHLY ON THE 1ST & 3RD SUNDAY OF THE MONTH WITH THE OPEN WATER DIVES UNDERTAKEN ON THE 2ND WEEKEND OF THE FOLLOWING MONTH. FOR EXAMPLE:
POOL SESSIONS: SUNDAY'S 2ND & 16TH MARCH
OPEN WATER DIVED: SATURDAY & SUNDAY 12/13TH APRIL
**All pool entry and equipment hire is included however you will be required to call into the dive centre at some point, prior to your 1st dive, to try on your course equipment as we do not keep any at the leisure centres or dive sites. It is also a requirement that your e-learning is completed in full prior to starting your open water dives.
With a wealth of combined knowledge and experience, our professional and friendly team are dedicated to ensure you not just enjoy and pass the course, but become great divers. You will receive your Open Water qualification once all parts have been successfully completed, qualifying you to dive THE WORLD, in depths up to 18m.
As soon as you purchase the course, we will issue you with a code for your e-learning, which you can begin at home & complete at your own pace. Approx 5-10 hours.
Our pool sessions are held at Poynton Leisure Centre, Yew Tree Lane, SK12 1PU, on Sunday afternoons from 3:00pm until 5:30pm.
Our open water dives are typically undertaken at The Delph Watersports Centre, Eccleston, over 2 days. Equipment hire is included for this, but please note that there there will be a site entry & membership fee requirement, payable upon entry to The Delph, of approx £50 for the weekend.

**This course is performance based so the times are approximate. If you require more training, in addition to the included time frame listed above, this may incur additional fees as follows: Confined Water +£45 per session, Open Water +£95 per session.

Booking and Expiry
- All courses must be booked and completed within the expiry date agreed upon at the time of purchase. No extensions will be granted under any circumstances.

Cancellations
- Cancellations must be notified at least 3 working days prior to the scheduled course date.
- Cancellations made within 3 working days of the scheduled course will result in the full course fee being forfeited.
- If you cancel or reschedule your course more than once, no further extensions will be provided, and the course fee will be forfeited.

Expiry and Rescheduling
- If you schedule your course for a date beyond the agreed expiry date and subsequently cancel, no further extensions will be provided, and the course fee will be forfeited.

Diver Medical Requirements
- All participants must read and fully understand the Diver Medical Questionnaire prior to booking.
- If you answer 'YES' to any questions in Sections 3, 5, or 10 of the questionnaire, a doctor’s note will be required.
- No refunds will be issued for failed medicals. However, at the owner’s discretion, your course fee may be applied toward other goods or services.

Late Arrivals
- All participants must arrive at the designated start time. Late arrivals will not be permitted to join the course, as resources and schedules cannot accommodate late starters.

E-Learning Requirements
- Participants required to complete e-learning must do so no later than 1 week prior to the course start date.
- Incomplete registrations or unfinished e-learning will result in exclusion from the course, with no refund or rescheduling allowed.

Additional Training
- Should you require additional training outside of the standard course schedule, this will incur additional charges as follows:
- £45 per person per pool session
- £95 per person per Open Water Dive

General Terms
- Course fees are non-refundable and non-transferable unless otherwise agreed at the owner’s discretion.
- No partial refunds or credits will be issued for incomplete courses.
